Form & Function provides fitness, Pilates, mobility, and strength training classes, either live, recorded, or on-demand, through our membership platform and related services. These services are for general fitness and wellness purposesand are not a substitute for medical advice, diagnosis, or treatment.

2. Eligibility

You must be at least 18 years old (or have parental/guardian consent if under 18) to use our services. By enrolling, you confirm you are healthy enough to participate in fitness activity and have consulted a physician if necessary.

3. Medical Disclaimer

  • Always consult with a healthcare provider before beginning any exercise program.

  • Stop immediately if you experience pain, dizziness, or shortness of breath.

  • Form & Function is not responsible for injuries or health complications that may result from participation.

4. Membership & Payments

5. Refunds & Cancellations

By participating, you voluntarily assume all risks related to physical activity. To the fullest extent permitted by law, you release and hold harmless Form & Function, LLC, its trainers, employees, and affiliates from any liability for injuries, damages, or losses that may occur.

8. Insurance

Form & Function carries professional liability insurance. However, this does not replace your responsibility to exercise within your own limits and consult your healthcare provider before starting or changing any fitness program.
